The absence of sperms in the semen of a man is termed as ‘Obstructive Azoospermia’. Men affected with this disease will face problems in sperm delivery. 40% of azoospermia cases today belong to obstructive azoospermia. In general, obstructive azoospermia causes due to the previous vasectomy. Obstructive azoospermia is a common cause of infertility in men and may cause due to congenital anomalies or iatrogenic injury, infection. Microsurgical vasal reconstruction is a preferable treatment for most cases of obstructive azoospermia.
Obstructive azoospermia is generally caused due to issues related to a ductal system and ejaculation. The ducts will get blocked or may be missing or may have a lack of the vas deferens leading to the obstruction in the tube which transports sperm for ejaculation.
Sometimes obstructive azoospermia is caused due to trouble with ejaculation. Mostly, there will be problems with discharge which is a procedure by which the sperm is deposited into urethra further on ejaculation. This is caused by neurological injury and spinal cord injury or from a surgery or diabetes.
Azoospermia can be diagnosed by examining the patient medical history, physical examination and by evaluating testes and scrotum or by conducting some laboratory test and imaging tests. Medical history of a patient includes general health, sexual health, libido sexual activity and past fertility. Other than these, the patient should be enquired about his past exposure to drugs or chemicals which include hormone and steroids, antibiotics, inhibitors like 5-ASA, recreational drugs like alpha blockers, excess alcohol, 5 alpha reductase inhibitors, pesticides, chemotherapeutic agents and high heat exposure of testes. The congenital absence of vas deferens is detected by physical examination and can be easily concluded by transrectal ultrasound (TRUS).
Azoospermia can be treated by surgery. Men suffering from obstructive azoospermia should undergo a surgery. This surgery will help in achieving natural pregnancy without the involvement of any reproductive technologies like IVF, ICSI, and IUI.
Vasoepididymostomy treatment is to be done in men who affected by an epididymal obstruction and Transurethral resection of ejaculatory duct (TURED) is suggestible for men who are affected with ejaculatory duct obstruction.
